A sports physical is a medical exam required by most schools, camps, and athletic programs before a student can participate in sports. Healthcare providers also call this a pre-participation physical evaluation (PPE).

How common are sports physicals?
Sports physicals are very common, especially for children and teenagers. In the United States, more than half of kids ages 6–17 participate in organized sports or athletic lessons.
Because so many students are active in sports, most schools, camps, and athletic organizations require a documented sports physical before participation. These exams help ensure young athletes are healthy and ready to play safely.
What can I expect during a sports physical exam?
A sports physical includes a review of medical history and a comprehensive physical exam to evaluate overall health and readiness for athletic activity.
Medical history review may include:
- Past injuries, illnesses, or surgeries
- Current medications or supplements
- Family history of relevant health conditions
The physical exam may include:
- Height and weight measurements
- Vital signs (blood pressure, heart rate, pulse)

- Heart and lung evaluation

When should I schedule a sports physical?
Healthcare providers recommend scheduling a sports physical 6–8 weeks before the sports season begins. This allows time to address any concerns before participation.
Most students need a sports physical every one to two years, or as required by their school or athletic program. Students recovering from injuries may need additional evaluations within the same year.
What’s the difference between a sports physical and an annual physical?
A sports physical focuses specifically on a student’s ability to safely participate in athletics.
An annual physical (well check) is more comprehensive. It reviews overall health, development, and preventive care, and may include immunization updates and broader wellness screenings.
Both exams are important, they simply serve different purposes.
What are the possible results of a sports physical?
After the exam, the provider may determine that the student is:
- Cleared to participate in all sports without restrictions
- Cleared for certain activities only
- Cleared with specific restrictions or recommendations
- Eligible to participate after further evaluation or treatment
- Not cleared due to health risks that require additional care
